Date of blacklist: I dont remember exactly, but I believe it was about the 27-28th April
Have you received any kicks/warns/bans, and if so, why: One ban for MRDM when I was new to the server
One warning for RDM for killing a d-class TB
One warning for NLR after I returned to a situation after death
Who blacklisted you from Staff: SL meeting
Why were you blacklisted from Staff: I was backlisted due to having no energy to finish my final weeks requirements. I had never missed a requirement beforehand and I was completely unmotivated to hop on for the final week.
Why should you return: Whilst I am aware that my blacklist has about a week left on its timer, I would like to appeal it so I have the option to reapply for staff again if I decide to reapply sometime soon. I recently returned to the server and I would like to have the chance to become staff sometime in the near future as I did enjoy my time as staff.
